On Hook = Yes and Two calls concurrently

vicibox 4.0.3, vicidial 2.8-404a, Build 130605-0841, asterisk 1.4.39.1-vici, Single Machine, Vtiger 5.4

At Admin --> Phone, On Hook = Yes, the problem that the agent is getting two calls at the same time. If I set call-limit = 1 then I will face problem that all the queue will be not organized (maybe the call is sent to the agent Phone and it will be refused because of call limit and that will effect on the calculations and coming back again the agent when the agent is available). So what is the solution?

agents should not get two calls at the same time regardless of the on hook setting. are both calls from the same campaign? is the agent logged in to two different sessions?
