Cuote for webform call?

I need a form when I enter just the number to call and the extension and when a dial bottom is press the phone ring with the call to pickup.
i'm using vicidialnow 2.0.5
asterisk vertion 1.2
and centos

"Manual Dial" on the agent screen in vicidial.
Or are you talking about a "call me now" button on a web page?

A webpage with 2 field and a Call Bottom
On field1 to enter phone: Phone Number
On field2 to enter the extention witch will ring: Exten Number
And 1 bottom to place the call
Just as simple as it sound , but I don't even know how to start, can you make me a coute for it??
