Error Loading Leads

I am using astguiclient-2.0.4. asterisk-14.X Below is my leads format in Excel .csv file

vendor_lead_code source_id list_id phone code phone title first name middle last name address1 address2 address3 city state province post code country gender birth date alt phone email security comments

1000 1 8568168996 Ken McNesby
1000 1 8566923558 Nellie Costanzo

Data is only there in the list_id, phone code, phone, first name, last name. Its giving me below errors.

How are you trying to load the leads?

CSV files usually have commas separating the fields
