No one availabe when transfering a call to Ext

I am facing an issue when agents transfer a call to Extension, but that doesn't happen all the time but sometimes we face this issue, the agents face a message that this extension is not available and routed to the extension's voicemail to leave a message when trying to call the ext they will transfer to, in order to transfer the customer to it.

here is the Asterisk output:

I guess this is what cause this issue, not sure:

and to transfer internally I did a dial plan to let me be able to record the extensions calls for only the transfer, not all the time, as below:

exten => _78XXXX,1,Dial(${TRUNKloop}/79${EXTEN:2},,To)
exten => _78XXXX,n,Hangup
exten => _79XXXX,1,Answer
exten => _79XXXX,n,Goto(defaultlog,${EXTEN:2},1)
exten => _79XXXX,n,Hangup

What's your setup? Single or cluster? How many agents with what dial ratio? I'm pretty sure it's from over stressing the system but answer those questions and I can assist you better.

It's single, and it's inbound calls, but the agents transfer some calls to support team and this is the issue and in this transfer, I am using the dial plan entry that I mentioned before

also currently the load is 57 - 2%, is that high?

No that's not high, it's a hard issue to debug off just info, I would need to see what's happening in Asterisk as I try different things. Message me on Skype and I'll try to assist you
