Cannot approve lead in QC queue

We are using the QC queue on a daily basis to approve (pass) or cancel sales. Now we have a sale that won't go away when we set it to the QCPASS result.

Does anyone have advice on how to troubleshoot this? Is there a log that we can check?

Never mind, we had a mysql trigger that was block the QCPASS status to be set.
